Add express-rate-limit to the billing checkout endpoint to prevent Stripe session spam and potential abuse.

Implementation:

• Install express-rate-limit if not present
• Create a rate limiter: 5 requests per 60 seconds per IP for POST /api/billing/checkout
• Return 429 with JSON { error: 'Too many billing requests' } when exceeded
• Add integration test: 5 requests succeed, 6th returns 429
• Apply only to this endpoint, not globally
