Add rate limiting middleware to the document generation endpoint to prevent abuse and control OpenAI API costs.

Implementation:

  • Install express-rate-limit package
  • Create rate limiter configured at 5 requests per 60 seconds per IP
  • Apply to POST /api/generate route only
  • Return 429 with structured error JSON (using apiError utility if available) when limit exceeded
  • Include Retry-After header in 429 response
  • Add X-RateLimit-Limit and X-RateLimit-Remaining response headers

Acceptance criteria:

  • POST /api/generate returns 429 after 5 requests in 60s from same IP
  • 429 response includes Retry-After header
  • Rate limit headers present on successful responses
  • Other endpoints are NOT rate limited
  • TypeScript compiles, tests pass
