Users without an active paid subscription can currently call POST /api/generate and get documents for free. This is a direct revenue leak.

Implementation:

1. Create src/middleware/require-subscription.ts (or extend existing auth middleware)
2. After authentication, query stripe_subscriptions table for the user's active subscription
3. Check status is 'active' (not 'canceled', 'past_due', or missing)
4. If no active subscription: return 403 with { error: 'Active subscription required', code: 'SUBSCRIPTION_REQUIRED' }
5. Apply this middleware to POST /api/generate route BEFORE the generation handler
6. Ensure the middleware passes subscription tier info downstream for usage limit enforcement (#1221)

Dependencies: Requires stripe_subscriptions table (PR #692) and billing webhook handlers (#1211, #1212) to keep subscription status current.

Acceptance: Unauthenticated users get 401. Authenticated users without active subscription get 403. Only users with active subscriptions can generate documents.
